To be considered for the Ambassador Program for Moments Photography by Carrie please fill out all parts of this form and email a jpeg format head shot of yourself to info@momentsphotographybycarrie.com. Please note that the head shot is required but will not be a determining factor in your acceptance into the program.

The Ambassador program is designed to promote Moments Photography by Carrie. Having an energetic and outgoing personality along with a positive attitude is most important.

I understand that Moments Photography by Carrie may use photographs from my session for displays, advertising, exhibitions, or competitions. I understand that there are several obligations as an ambassador such as deadlines and appointments, and I will perform these duties to the best of my abilities. Any violation of this agreement will terminate any credits and program benefits that I am entitled to from referrals.
